Download the SYEP Application here.  Once completed, please bring with you the SYEP Application and the following documents to our office Monday through Friday from 9:30 am to 4 pm:

 

• A Copy of your Birth Certificate or Alien Registration Card

• A copy of your Social Security Card

• Photo ID, Driver's License or Non-Driver ID card

• Working papers

• Proof of disability, if applicable

• Proof of income (for more information, please refer to TANF Poverty Guidelines here)

• Resume

Please note:

• Youth who are 18 years old or younger and are currently attending high school must be accompanied by a parent or guardian at the time of submitting the SYEP Application.

• If you are found eligible for the program, you will have to get a physical examination and pass a drug test.
