Summer Youth Employment Program Business Inquiry Form

Each year, the Town of Hempstead Department of Occupational Resources enacts an employment program for serving eligible youth, ages 16-21, who are low-income and face a barrier to education, training and employment.  For 6 weeks each summer, youth are placed in employment as interns at various worksites.  Youth are paid the minimum wage and may work up to 30 hours a week.  Please complete the form below if you would like youth to work at your organization during the summer.

3. Has the establishment been the subject of a Federal, State, or City investigation, criminal, or civil action?
4. Is the worksite child-care related?
5. Will youth participants be instructed to work outside?
6. Will youth participants work on community service projects?
7. Will youth participants be instructed to handle dangerous tools (e.g. power tools or knives)?
